Scripts and reasoning in medicine: theory, assessment and the development of reflectivity

Summary: According to script theory, health professionals progress toward clinical problems solutions with hypotheses or options and their related knowledge networks (scripts) in mind. They actively use them to constantly make judgments on the effect each new piece of information has on the status of the hypothesis or option. Each of these judgments can be measured. The script concordance test is built on these premises. It allows assessment of clinical reasoning in the complexity of real clinical settings. The approach is based on three principles: (1) Examinees are faced with challenging authentic situations in which several options are relevant; (2) The response format is a Likert scale that reflects the way information is processed in problem-solving situations, according to the script theory; (3) Scoring is based on the aggregate scoring method to take into account the variability of reasoning processes among experts. Research findings are presented. They suggest that the approach permits to reliably discriminate examinees across their level of experience, and so in very different domains. It makes it possible to measure skills or domains that were up to now difficult to measure.
